Why Is Health Insurance So Expensive

The driving factor for why health insurance is so expensive is that health care is so expensive, says Louise Norris, a licensed health insurance agent based in Colorado and author of The Insiders Guide to Obamacares Open Enrollment. The price of health care in this country is really high.

According to a 2020 report from the Kaiser Family Foundation, insurers said the reasons they had to increase premium costs included the continued cost of COVID-19 testing, the rebounding of medical services that had been delayed during the pandemic and morbidities related to foregone care.

How Health Insurance Works

Health insurance can be tricky to navigate. Managed care insurance plans require policyholders to receive care from a network of designated healthcare providers for the highest level of coverage. If patients seek care outside the network, they must pay a higher percentage of the cost. In some cases, the insurance company may even refuse payment outright for services obtained out of network.

Many managed care plansfor example, health maintenance organizations and point-of-service plans require patients to choose a primary care physician who oversees the patient’s care, makes recommendations about treatment, and provides referrals for medical specialists. Preferred-provider organizations , by contrast, don’t require referrals but do have lower rates for using in-network practitioners and services.

Insurance companies may also deny coverage for certain services that were obtained without preauthorization. In addition, insurers may refuse payment for name-brand drugs if a generic version or comparable medication is available at a lower cost. All these rules should be stated in the material provided by the insurance company and should be carefully reviewed. It’s worth checking with employers or the company directly before incurring a major expense.

How Health Insurance Deductibles Work

Vision Insurance Las Vegas NV

Typically, deductibles are met on an annual basis, meaning they reset at the beginning of each year. âLetâs say your total medical care for the year equaled $3,000 and your annual deductible is $1,000. In this scenario, you would pay $1,000 out of pocket, and your insurance benefits would then cover the remaining $2,000,â explains Dr. Johnston.

She notes that if coinsurance is involved, the remaining amount beyond the deductible is shared between you and the insurance company. âThe industry standard for coinsurance is 20% for the insured and 80% for the insurer,â she says.

Imagine your coinsurance is 20%. You go to the doctor, and your planâs allowed amount for an office visit is $100. If youâve already met your deductible, you pay 20% of $100, or $20. If you havenât, you pay the entire allowed amount of $100.

A 2020 study suggests deductibles come with both benefits and drawbacks. While they lead to a decrease in the use of various services and tend to be profitable for young and healthy people, they also result in higher of out-of-pocket costs and hospitalization rates over time.

Example Of A First Dollar Car Insurance Policy

Assume that a driver damages their car when driving into the garage. They take the car to a body shop and the damage is estimated at $3,000. Typically, in this situation, the person may opt to pay to fix the damage themselves, or if they can’t afford the upfront cost they may file an auto insurance claim. Filing a claim usually involves paying a deductible, such as $250, $500, or $1,000 depending on the insurance policy. Lower deductibles typically have higher insurance premiums.

The insurance company covers the cost, but the driver is also paying some of it by way of the deductible. If the deductible is $500, they are sending $500 to the insurance company and receiving $3,000 for a net inflow of $2,500 to cover the damage costs.

Assume the driver has a first dollar car insurance policy. This means they don’t need to pay the deductible. The insurance company pays the full amount, and the driver receives the $3,000. At first glance, the driver is better off with this policy since their net inflow to cover accident costs is greater.

When a member has double insurance, his or her individual circumstances determine which insurance is primary and which is secondary. Following are some examples of how this might work:

    • A married couple A wife has a health plan with her employer, but her husbands health plan also covers her. In this case, the wifes employer is the primary insurer and the spouses health plan is secondary.
    • A child under 26 The Affordable Care Act lets children stay on their parents health plan until they turn 26. That could result in a child having her own health plan through an employer while remaining on the familys plan. In that case, the childs health plan is primary and the parents plan is secondary.
    • Parents have separate plans and a child is on both plans In this situation, the so-called birthday rule applies. Whichever parent has the earlier birthday in a year is considered the primary health plan and the other spouse is secondary. Its not which parent is older. Instead, its which one has the earliest birthday in a calendar year.
    • Medicare and a private health plan Typically, Medicare is considered primary if the worker is 65 or older and his or her employer has less than 20 employees. A private insurer is primary if the employer has 20 or more employees.

Health insurance can get confusing. Heres a quick breakdown of plans that are typically available through networks:

    • Exclusive Provider Organization . This is a managed care plan where services are covered only if you use doctors, specialists, or hospitals in your plans network, except in an emergency.
    • Health Maintenance Organization . An HMO usually limits coverage to care from doctors who work for or contract with the HMO. It generally wont cover out of network, except in an emergency. The plan may require you to live or work in its service area to be eligible for coverage.
    • Point of Service . With this plan, you may pay less if you use doctors, hospitals, or other healthcare professionals that belong to the plans network. They typically require you to get a referral from your primary care doctor to see a specialist.
    • Preferred Provider Organization . You pay less if you use providers in the plans network. You can, however, use doctors, specialists, and hospitals that are out of network without a referral for an additional cost.

Major Considerations For Health Insurance Deductibles

âFor people who have enough savings to pay their deductible in the event of an emergency or who are relatively healthy, a high deductible plan may be cheaper annually due to lower monthly costs. For people with medical conditions that require frequent care, a lower deductible with a higher monthly cost makes sense,â says Bethany Malone, M.D., a colon and rectal surgeon at Fort Worth Colon and Rectal Surgery Associates in Fort Worth, Texas.

Of course, donât be afraid to look around. âItâs wise to shop around and compare different plans and their respective deductibles, copays, premiums and prescription costs so you can find the right one for your unique situation,â says Dr. Sanghavi.

    Primary And Secondary Insurance Rules

    Medicaid Insurance Companies at HF

When you have two forms of health insurance coverage, your primary insurance pays the first portion of the claim up to your coverage limits. Your secondary insurance may pick up some or all of the remaining costs.

However, you still might be responsible for some cost-sharing. For example, its a mistake to think your secondary insurance will kick in and cover the deductible attached to your primary insurance. Instead, you likely will be responsible for covering the deductible.

You also may be responsible for copay and coinsurance fees.
