How Much Lower Would Prescription Costs Be

Estimated reduction in drug spending
friedman

Patients in the United States pay the highest prices in the world for prescription drugs. Thats partly a result of a fractured system in which different payers negotiate separately for drug benefits. But it also reflects national preferences: An effective negotiator needs to be able to say no, and American patients tend to want access to the widest array of cutting-edge drugs, even if it means paying more.

A Medicare for all system would have more leverage with the drug industry because it could bargain for the whole countrys drug supply at once. But politics would still be a constraint. A system willing to pay for fewer drugs could probably get bigger discounts than one that wanted to preserve the current set of choices. That would mean, though, that some patients would be denied the medications they want.

All of our economists thought a Medicare for all system could negotiate lower prices than the current ones. But they differed in their assessments of how cutthroat a negotiator Medicare would be. Mr. Friedman thought Medicare for all could reduce drug spending by nearly a third. The Urban team said the savings would be at least 20 percent. The other researchers imagined more modest reductions.

Coinsurance And The Metal Tiers

Your coinsurance percentage depends on the details of your individual insurance policy. If you got a plan through the, then your plan falls into one of four tiers â Bronze, Silver, Gold, Platinum. These are called the metal tiers. The tier a plan falls into depends on how the insurer will split all costs with you, which isnât the same as your coinsurance split.

With aBronze plan, for example, insurers cover an average of 60% of your medical costs, leaving you to pay 40%. The 60/40 cost sharing factors in copays, coinsurance, and the costs you will pay before and after hitting your deductible. So the average cost-sharing value for the tier of your insurance plan may not be the same as your coinsurance percentage. In fact, itâs possible to have a plan with 0% coinsurance, meaning you pay 0% of health care costs, or even 100% coinsurance, which means you have to pay 100% of the costs.

The following table lists the general cost-sharing percentages for each of the metal tiers. Bronze plans require you to pay the most while the Platinum plans require you to pay the least. At the same time, Bronze plans usually have the lowest monthly premiums and Platinum plans usually have the highest premiums.

Why Do Us Health Insurance Costs Keep Going Up

The cost of US health insurance has almost doubled in a decade, as the table shows:

2010
$5,588

Whats behind this trend is the subject of debate some argue government programmes, such as Medicare and Medicaid , have relieved providers of pressure to keep insurance affordable.

However, its likely that some factors driving up the cost of healthcare in the USA may be similar to those in other countries, including population ageing and an increase in chronic illness. Check out how much medical treatment can cost abroad.

At the same time, a shift towards high-deductible health plans is increasing the out-of-pocket costs for Americans. Under such plans, families can be asked to pay for their first $14,000 of medical costs, so the impact may be greater even if their insurance package costs the same overall.

What Are The Specific Costs Involved

There are a number of costs associated with individual health insurance that you may have to pay, depending on the type of policy you have.

  • A premium is the amount you pay your insurance carrier each month.18
  • A deductible is the amount of money you must pay out of your pocket before your insurance begins to pay a benefit.19
  • A copayment is a flat dollar amount you may have to pay for certain services
  • Coinsurance is a percentage of covered health services that you are responsible for paying for once youve met your deductible.20
  • An out-of-pocket maximum is the total amount of money you may have to pay for healthcare services in a particular plan year. 21

In some cases, a person may have to pay each of these costs. For example, a consumer may have a healthcare plan with a $1,000 deductible that costs $300 in monthly premiums. After hes met his deductible, he may have to pay $50 for a doctors office visit and 30% of the cost for any laboratory services. The good news is this consumer knows his total out-of-pocket costs will be capped by his out-of-pocket maximum in case he becomes seriously ill or injured.

How Much Would Doctors And Hospitals And Other Providers Be Paid

Estimated increase in Medicare payment rates paid to medical providers

In our current system, doctors, hospitals and other health care providers are paid by a number of insurers, and those insurers all pay them slightly different prices. In general, private insurance pays medical providers more than Medicare does. Under a Medicare for all system, Medicare would pick up all the bills. Paying the same prices that Medicare pays now would mean an effective pay cut for medical providers who currently see a lot of patients with private insurance.

For a Medicare for all system to save money, it needs to reduce the health care industrys income somewhat. But if rates are too low, hospitals already facing financial difficulties could be put out of business.

Neither Mr. Sanderss legislation nor the Jayapal House bill specify what the Medicare for all system would pay, but they say that Medicare would establish budgets and payment rates. So our estimators offered their best guess of what they thought such a plan might do.

How Much More Would People Use The Health Care System

Estimated increase in use of health care
friedman
8%

Medicare for all would give insurance to around 28 million Americans who dont have it now. And evidence shows that people use more health services when theyre insured. That change alone would increase the bill for the program.

Other changes to Medicare for all would also tend to increase health care spending. Some proposals would eliminate nearly all co-payments and deductibles. Evidence shows that people tend to go to the doctor more when theres no such cost sharing. The proposed plans would also add medical benefits not typically covered by health insurance, such as dental care, hearing aids and optometry services, which would increase their use.

The economists differ somewhat in how much they think people would increase their use of medical services.
