How To Read Your Health Insurance Card
Most insurance cards in the U.S. contain the same basic information, including:
The name of your insurance company and contact information such as a customer service phone number, email address and website.
The type of plan you have .
Your name and the name of any dependents covered by your policy. If you are the dependent on someone elses policy, their name will probably be on your card.
Your insurance I.D. number on the insurance card indicates the coverage your plan provides. If youre the policyholder, the last two digits in your number might be 00, while dependents might have numbers ending in 01, 02, etc.
An insurance group number, if you have an employer-based plan. If you purchase private insurance through healthcare.gov, a state exchange, or directly from an insurance company, there might not be a group number on the insurance card.
The amount of your or for common situations, such as a visit to your primary care doctor, a specialist, an emergency room or an urgent care clinic. A copay usually appears as a flat rate, a coinsurance as a percentage. Your card may also list the prices for in-network providers and out-of-network care.

Removal Of Sex Designation
Beginning February 11, 2019, Nova Scotia residents will have the option to remove their sex designation from the front of their health card. There is no fee for this service.
If you choose to remove your sex designation from the front of your card, you will still be able to access insured physician and hospital services.

Always show your new member ID card to health care providers so they can quickly access your health plan benefits and help you get the most value from your plan. - This can be a death certificate from the funeral home or a certificate from ServiceOntario

A PEI Health Card is issued to every PEI resident. You are considered to be a resident of PEI if:
- you are legally in Canada and
- PEI is your primary residence for at least six months plus a day each year.
You are not eligible for a PEI Health Card, if you are:
- an International student, unableto work off campus
- a tourist, transient or visitor to PEI
- a member of the Canadian Armed Forces

When Your Card Goes Missing
The first thing to do when your card is lost, for whatever reason, is to call your state Medicaid office as soon as possible. You can find a comprehensive list of state Medicaid contacts on the Medicaid.gov website. Call to speak with a representative, or visit your states Medicaid website, to report a lost or stolen card. Be prepared to give your identifying information, including your Social Security number, when you call.

Your Health Insurance Card
After you enroll in a private health plan and pay your first month’s premium, you will receive a membership package with your insurance card and summary of benefits and coverage detailed information.

What If I Lost My Health Card
If your PEI Health Card is lost or stolen, you can apply for a replacement card. The replacement fee is $10 per card, with a household maximum $50.

Q My Baby Was Born In Hospital Or At Home Attended By A Registered Midwife How Do I Get A Health Card For My Baby When He Or She Is Born
There is a special registration process for babies born in Ontario birthing hospitals and for babies born at home attended by a registered midwife.
Immediately after the birth of your baby, hospital staff or a registered midwife will give you an Ontario Health Coverage Infant Registration form to complete. Babies born in an Ontario birthing hospital or at home and attended by a registered midwife will be provided with an infant registration form. Tourists, transients or visitors are not eligible for OHIP coverage.
This form requests information regarding your baby such as the babys name, birth date and mailing address. Information is also requested regarding the parent/guardian who is completing the form. You will be asked to confirm that :
